Please fill out one slip per student,along with the enrollment forms. Return to the school on or before the 1st day of school.

Instructions: If you have more than one student enrolling in our district please note the following: One enrollment packet per student will need to be completed. Regarding the lunch application; one application per household will need to be completed (if you have more than one student enrolling, you will only need to complete the lunch application only once) All forms along with birth certificate, immunization record, social security card and verification of residency in the district (electric bill, phone bill, etc.) will need to be shown to the school office upon registering your student.
